Cancellation Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the providers’ day that could have been filled by another patient. As such, we require 24 hours’ notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be subject to a $50 cancellation fee charged to the card on file.

Important Payment Information
Please note that we do not accept insurance, Medicare, Medicaid, or HSA/FSA for our services. We are committed to providing the highest quality care and treatments, and we appreciate your understanding and cooperation.

Return & Refund Policy
At Revive Skin Clinic & Aesthetics, we offer goods and services that are irrevocable. Therefore, we do not issue refunds for any product or service. In consenting to be treated, it is important that patients understand and accept this condition.
Once services are purchased they will not be refunded, however, to ensure our clients always receive the greatest experience at Revive Skin Clinic & Aesthetics, unused service values (cash equivalent for the remaining amount of a treatment package) can be applied to any other service at Revive Skin Clinic & Aesthetics.
For Skin Care Products, all sales are final, however, should you have a skin reaction to one of the products, it can be returned for a full refund within 7 days of purchase with manager’s approval.
All treatments, procedures, and or pre-paid packages are non-refundable. Packages are non-transferable and do not have an expiration to use the services. Credit from pre-paid treatments, procedures, and or packages may be applied toward other forms of treatments(s) or product(s) only at management’s discretion.
All injectable treatment sales (such as, but not limited to Botox, Dysport, Jeuveau, Xeomin, Sculptra, dermal fillers), laser treatments, facial treatments, or chemical peels are final; refunds or credits cannot be offered once treatment is completed.

Chargeback Policy
Upon payment for services rendered, the charges are final and non-refundable. Any disputes or chargebacks through the purchaser’s credit card issuer will not be accepted after services have been provided. If a chargeback is initiated or the charge is disputed, the purchaser will be held responsible for any fees, costs, or legal expenses incurred by Revive Skin Clinic & Aesthetics, PLLC in the process of resolving the dispute.
In the event of a chargeback, treatment may be withheld, and any further procedures may be contingent upon payment of outstanding balances.
We will be honest in all our dealings with you. Aesthetics is not an exact science, and how you may respond to a given treatment will vary from person to person. It is virtually impossible to predict results, and therefore payments made for services are for treatments to be performed -- not for a specific result. However, we always strive to achieve the absolute best result that we can for you.

Age Policy
No Children Under 12 Policy
At Revive Skin Clinic & Aesthetics, our priority is to maintain a safe, relaxing, and professional environment for all clients and staff. To ensure the highest standard of care and safety, we kindly ask that no children under the age of 12 be brought onto the premises.
For safety reasons, children are not permitted in treatment rooms or waiting areas.
Our facility is not designed to accommodate young children, and we are unable to provide supervision.
Clients arriving with children under 12 will be asked to reschedule their appointment.
We appreciate your understanding and cooperation in helping us create a serene and secure environment for everyone.

Pre-Paid Service & Package Expiration Policy
At Revive Skin Clinic & Aesthetics, we are committed to providing the highest level of care and service to our valued clients. To ensure a seamless experience, please note the following policy regarding pre-paid services and treatment packages:
All services purchased as part of a package or otherwise pre-paid must be scheduled and redeemed within one (1) calendar year from the original purchase date.
Any unused services remaining after this period will expire and will no longer be valid for redemption.
Extensions may be considered in exceptional circumstances at the sole discretion of management.
We encourage clients to schedule their appointments in advance to maximize their benefits and ensure availability. If you have any questions, please contact our team for assistance.
